Brophy Curved 20cm — Secondary Cleft Revision Surgery
The 20cm-labelled curved Brophy serves secondary cleft revision surgery — the touch-up operations performed at age 4-6 and again at age 14-18 to address residual aesthetic and functional deficits from the primary cleft repair. Secondary surgery is technically demanding because the operative field contains scar tissue from previous operations, the anatomic landmarks are distorted, and the cosmetic margin for error is small.
The secondary-revision indications
Common indications include vermillion-border misalignment, residual nasal-tip asymmetry, persistent oronasal fistula, velopharyngeal insufficiency requiring secondary pharyngoplasty, and Le Fort I osteotomy for midface retrusion (typically performed at skeletal maturity in late adolescence). Each indication requires precise handling of scarred tissue at the corner of the mouth or the lip margin — the curved 20cm Brophy is the right tool for this work.
The lifetime-care cleft model
Modern cleft care follows the patient from infancy through adulthood with planned interventions at developmental milestones; secondary revision surgery accounts for 40% of total surgical procedures in the cleft pathway. The curved 20cm Brophy lives in the cleft-team operating tray throughout the patient’s lifetime of care.
