Crafoord Tonsil Scissors – Angled Scissors for Tonsillar Fossa Dissection and Tonsillectomy

The Crafoord Tonsil Scissors are named after the Swedish cardiovascular surgeon Clarence Crafoord (1899–1984), who also contributed to head and neck surgical instrumentation. These scissors feature distinctively angled blades (approximately 25–30 degrees offset from the shank axis) with semi-blunt tips, measuring 18–20 cm in overall length. The angular blade design provides unobstructed access to the tonsillar fossa and peritonsillar space while the surgeon’s hand remains clear of the oral cavity, preventing visual obstruction during tonsillectomy.

The blades are medium-weight with beveled cutting edges designed to transect the tonsillar capsule, peritonsillar connective tissue, and palatoglossus/palatopharyngeus muscle fibers encountered during extracapsular tonsillectomy. The semi-blunt tip configuration allows blunt dissection and spreading within the peritonsillar space to develop the correct tissue plane before sharp division, reducing hemorrhage from the extensive tonsillar vascular pedicle. Crafoord scissors are the preferred instrument in many European ENT departments for cold dissection tonsillectomy, which has been shown to produce less postoperative pain than electrocautery techniques.

Key Features

  • AISI 420 Martensitic German-grade Stainless Steel: Hardened blades provide reliable cutting performance through dense tonsillar capsule and peritonsillar connective tissue.
  • 25–30° Angled Blade Design: Offset blade angle provides direct tonsillar fossa access without hand obstruction of the oropharyngeal surgical field.
  • Semi-Blunt Tip Configuration: Rounded tips permit safe blunt dissection in the peritonsillar plane before sharp tissue division, reducing hemorrhage risk.
  • 18–20 cm Working Length: Sufficient reach for posterior tonsillar pole access through the opened oral cavity without compromising handle control.
  • Medium-Weight Beveled Blades: Balanced blade thickness for both cutting and spreading maneuvers during combined blunt-sharp tonsil dissection.

Clinical Applications

  • Extracapsular Tonsillectomy: Cold dissection of palatine tonsils from the tonsillar fossa along the peritonsillar capsule plane.
  • Peritonsillar Abscess Drainage: Incision and dissection of quinsy (peritonsillar abscess) with controlled drainage of the peritonsillar space.
  • Adenotonsillectomy: Combined tonsil and adenoid removal in pediatric patients with obstructive sleep-disordered breathing.
  • Tonsillar Fossa Biopsy: Excisional biopsy of suspicious tonsillar lesions including asymmetric tonsillar hypertrophy for lymphoma workup.
  • Parapharyngeal Space Access: Dissection through the tonsillar fossa to access parapharyngeal tumors via the transoral approach.
