Freer Septum Elevator — Blunt-Only Wide Variant
The blunt-only Freer is the elevator for virgin tissue: a wide working face that lifts perichondrium off cartilage in long clean strokes, with no sharp end to catch the flap or score the cartilage in the operator’s non-dominant hand. The wider face matters in a routine first-time septoplasty where most of the elevation is straightforward and time on the flap is the limiting factor.
The teaching argument
Some training programmes teach septoplasty with the blunt-only Freer specifically because it forces the operator to find the correct subperichondrial plane before any cutting begins — a sharp instrument lets a beginner power through the wrong plane and tear the flap. Once the plane is mastered the surgeon graduates to the double-ended classic; some senior surgeons go back to the blunt-only because of how cleanly it works in the typical case.
