How the Kerrison Rongeur Works
The instrument has two components: a footplate (the lower jaw) that slides under the bone structure to be removed, and an upper cutting punch that drives down to meet the footplate and cuts out a precise “bite” of tissue. The action is similar to a paper hole punch — the footplate protects underlying dura and neural structures while the punch removes bone cleanly without crushing.
This controlled cutting mechanism is what makes Kerrison rongeurs safer than chisels or drills in proximity to the spinal cord and nerve roots. Each bite removes approximately 2–3 mm of tissue, and the surgeon works bite by bite to achieve the required decompression.
Jaw Angle: The Critical Selection Parameter
Kerrison rongeurs are described primarily by jaw angle — the angle between the footplate and the instrument shaft. The angle determines which approach direction the instrument can work from:
- 40° (up-angled): The most common angle for standard laminectomy and discectomy. Cuts upward, suitable for approaching from below the structure to be removed.
- 90° (side-cutting): Used when the approach is lateral to the target tissue. Common in foraminotomy where the lateral recess needs to be widened.
- 130° and 180°: Steep-angle instruments for working in highly restricted spaces or for addressing structures on the contralateral side without repositioning the retractor system.
Bite Width: What the Size Numbers Mean
Kerrison rongeurs are available in bite widths from 1 mm to 6 mm, with 2 mm and 3 mm being the most commonly used sizes in spinal decompression. Smaller bite widths (1–2 mm) are used for delicate work near the dura or for fine foraminal decompression. Larger bite widths (4–6 mm) allow faster bone removal in open approaches where precision is somewhat less critical than speed.
Handle Designs
Standard Handle
Ring-and-ratchet handle style. The surgeon squeezes the rings and the footplate-punch mechanism closes. Standard handles are familiar to all spinal surgeons.
Springback Handle
Has a spring return mechanism that reopens the jaws automatically after each bite, reducing repetitive strain in long decompression procedures. Springback handles are increasingly preferred for procedures requiring many sequential bites.
Bayonet Handle
An offset handle design that keeps the surgeon’s hand out of the operative field when working in deep, narrow exposures. Common in minimally invasive spinal approaches.
Detachable vs Non-Detachable Footplates
Kerrison rongeurs with detachable footplates allow the footplate to be replaced when it becomes worn without discarding the entire instrument — a meaningful cost saving over the instrument’s lifespan. Non-detachable footplates are less expensive initially. For high-volume spinal surgery programs, detachable footplate models have a lower total cost of ownership.
