Optimal cytoreduction (residual disease <1cm) is the most important prognostic factor in advanced ovarian-cancer surgery, more significant than chemotherapy regimen or patient-baseline characteristics. The procedure can be extensive — total abdominal hysterectomy with bilateral salpingo-oophorectomy, omentectomy, pelvic and paraaortic lymphadenectomy, peritoneal stripping, bowel resection, and (in selected cases) splenectomy or diaphragmatic peritoneal stripping.
