20 cm is the deepest standard length — open-heart aortic-valve replacement annulus-suture trim, posterior-mediastinal node-dissection during oesophagectomy, and trans-hiatal oesophageal pull-through stitch cut. At this length the surgeon stands away from the wound and accepts longer-blade dynamics in exchange for visualisation.
