Mayo-Hegar Needle Holder
The most widely used design worldwide. The Mayo-Hegar uses a standard ring-handle with a ratchet lock that clicks into position to hold the needle without hand effort. The jaws are cross-hatched; premium versions have tungsten carbide (TC) inserts that grip smaller needles without letting them rotate.
When a surgeon says “pass me a needle holder,” they nearly always mean a Mayo-Hegar.

Mathieu Needle Holder
A fundamentally different handle design. The Mathieu uses a palm-grip mechanism: the instrument stays closed under spring tension and opens when you squeeze. Releasing pressure locks it. No ring handles.
The ergonomic benefit is real. Surgeons who do high-volume suturing report less hand fatigue with the Mathieu because the palm grip demands less intrinsic muscle effort than ring handles during repetitive closing. The trade-off is a learning curve — some surgeons find the locking action counterintuitive under time pressure.
Mathieu holders are popular in plastic surgery, gynecology, and any specialty where suture count per case is high. They handle needle sizes from 10 mm to 26 mm. Not the right choice for very large needles used in mass closure.
Crile-Wood Needle Holder
Similar to Mayo-Hegar in handle design but with a longer, lighter jaw profile. The narrower jaw makes the Crile-Wood well-suited to suturing in tight anatomical spaces where the bulk of a Mayo-Hegar jaw would obstruct visibility or access. It appears often in pediatric surgery trays and in cases where fine suturing is required in a confined field.
Castroviejo Needle Holder
Spring-loaded, no ring handles, very fine jaws. The Castroviejo was developed for ophthalmology and remains the standard for microsurgical suturing. It works with 7-0 through 10-0 monofilament sutures. Useful in hand surgery and microvascular anastomosis as well. Not appropriate for sutures heavier than 6-0 — the delicate jaws would be damaged.

Tungsten Carbide vs Standard Stainless Jaws
TC inserts last three to five times longer than standard stainless jaws before requiring replacement. For hospitals with high suturing volume, the cost premium — typically 15–25% over standard models — returns quickly in reduced downtime and instrument replacement frequency.
