Three scopes, one anatomical corridor, and a surprising amount of confusion in purchasing departments — partly because suppliers use the names loosely, and partly because the proctoscope sits in an awkward middle ground that some specialties skip entirely. Here is how the three instruments actually divide the territory.
Anoscope: The First 8 Centimeters
The anoscope is a short, beveled or slotted speculum for the anal canal itself. Push it past 9 cm and there is nothing to see — the instrument is designed to evert and display the canal walls, hemorrhoidal cushions, and dentate line under direct vision.
Pattern choice matters more than length here:
- Full-circumference (Hirschman type) — 19, 21, 23 mm diameters with obturator; the general-purpose diagnostic pattern, usually with a chrome or matte interior and a proximal light carrier
- Slotted (Chelsea-Eaton, Sims-type) — an open side channel that lets a hemorrhoidal cushion prolapse into the slot for banding or sclerotherapy; the operative pattern
- Fenestrated with oblique bevel (Kelly) — the bevel presents one wall at a time, useful for fissure inspection
No bowel preparation is needed, which is why the anoscope lives in outpatient clinics rather than endoscopy suites.
Proctoscope: The Rectal Ampulla
At 13–15 cm, the proctoscope traverses the full rectum. The classic stainless pattern (Naunton Morgan and similar) is a straight tube with obturator, proximal viewing window, insufflation port, and a fiber-optic or bulb light carrier. It earns its place for three jobs the anoscope cannot do: punch biopsy of rectal lesions under direct vision, assessment of proctitis extent, and retrieval of low rectal foreign bodies with long grasping forceps passed through the lumen.
One point of chronic naming confusion: in much of the literature, “proctoscopy” and “rigid sigmoidoscopy” are used interchangeably, and some manufacturers label a 25 cm instrument a proctoscope. When ordering, specify working length and diameter, not just the name on the box.
Rigid Sigmoidoscope: 25 cm and Beyond
The rigid sigmoidoscope (25 cm standard, 35 cm long pattern; 15–20 mm bore) reaches through the rectosigmoid junction into the distal sigmoid. In practice, the average depth achieved is 17–20 cm — the acute rectosigmoid angle stops the tube in many patients, which is precisely why flexible sigmoidoscopy took over screening work. The rigid instrument survives in specific niches: accurate distance measurement of rectal tumors from the anal verge (the rigid scale is more reliable than a flexible scope’s), sigmoid volvulus decompression with a flatus tube, and settings without endoscope reprocessing capacity.
A complete rigid set includes the scope, obturator, glass viewing window, insufflation bellows, light carrier, and long biopsy forceps (Chevalier Jackson pattern, 35–45 cm).
Choosing Between Them
The honest purchasing logic for a clinic or hospital in 2026:
Every clinic doing anorectal work needs anoscopes — at minimum one full-circumference diagnostic pattern and one slotted operative pattern, in two diameters. They are inexpensive, autoclavable, and have no flexible-scope substitute for hemorrhoid procedures.
Proctoscopes belong where rectal biopsy happens without immediate endoscopy access: surgical outpatient departments, rural hospitals, field units.
Rigid sigmoidoscopes are justified for colorectal units (tumor height measurement) and emergency departments covering volvulus. For screening, flexible instruments replaced them a generation ago.
Reusable stainless patterns dominate all three categories outside high-volume Western outpatient clinics, where single-use clear polymer anoscopes have taken share. Light Delivery: The Detail That Dates an Instrument
All three scopes are only as good as their illumination, and this is where instrument generations show. The oldest patterns used a distal tungsten bulb on a slide-in carrier — serviceable, but bulbs dim, heat the tissue, and fail mid-examination. The current standard is a fiber-optic light carrier: a removable stainless conduit that couples a standard endoscopy light cable to the scope’s rim, throwing cold light down the lumen with nothing electrical near the patient. When purchasing, confirm the carrier fitting matches your existing light source connectors (ACMI, Storz, and Wolf fittings are not interchangeable without adaptors) — a mismatch here is the single most common reason a new scope sits unused in its case.
Battery-handle illumination, borrowed from the laryngoscope world, occupies the middle ground for field and outreach work where no light tower exists. It adds weight at the proximal end but makes the instrument fully self-contained.
Sizing Beyond the Standard Adult
Pediatric anorectal examination uses narrower instruments — anoscopes at 10–15 mm diameter and proportionally shorter — and infant patterns smaller still, used in the assessment of anorectal malformations. At the other end, operative anoscopes for stapled hemorrhoidopexy run to 33–37 mm circular patterns supplied with the stapler system rather than the general tray. A department’s realistic minimum inventory: two adult diameters of each diagnostic pattern, one slotted operative anoscope, and pediatric sizes matched to the unit’s caseload.
Materials and Reprocessing
Reusable scopes are drawn AISI 304 stainless tube, mirror-polished internally for light reflection, with brass-chromed fittings on older patterns. All-stainless construction autoclaves at 134 °C without issue; instruments with cemented glass windows or bulb-holder assemblies need the window assembly removed before steam sterilization. Inspect the distal rim every cycle — a burr on the rim from a dropped scope will do exactly what you imagine to the anal canal. - Working length and diameter stated numerically on the purchase order — never rely on the pattern name alone
- Obturator included and matched — an obturator from another manufacturer’s scope will either rattle or jam; each tube ships with its own
- Light carrier fitting specified (ACMI/Storz/Wolf) against your existing cables
- Interior finish — mirror-polished lumen for bulb/fiber illumination; matte exteriors reduce glare at the rim
- Insufflation port with functioning valve on proctoscopes and sigmoidoscopes — test before first use, not during it
- Biopsy forceps length at least 10 cm longer than the scope it serves
