Cusco Side-Screw Virgin Speculum — 75 × 17 mm
The virgin-size Cusco is built to the dimensions of a hymen-intact introitus: 17 mm closed-blade width fits through an unstretched ring without tearing, and the 75 mm length reaches the cervix of an adolescent or nulligravid adult without pushing through the posterior fornix. It exists because the standard adult speculum is the wrong tool for an examination in which the patient’s first concern is preserving anatomy that has not yet been disturbed.
When to reach for it
Indications include investigation of primary amenorrhoea when imperforate hymen has been excluded but visual inspection of the cervix is required, evaluation of vaginal foreign body in paediatric gynaecology, post-menopausal atrophic vaginitis where the standard adult blade is unbearable, and any case in which a patient (or guardian) has explicit consented to a minimum-trauma exam. In cultural settings where preserving the hymen has documentary or testimony implications, the 17 mm width and the operator’s pre-procedure briefing both matter.
The side-screw on a narrow blade
Even at 17 mm closed width the lateral screw locks the chosen aperture, so the operator can release the handle during a brief swab or visual check; the patient is not held open for longer than the examination requires.
