Most procurement failures are visible in that fourth column. Departments buy on unit price and then discover the constraint was never price — it was how many sets they needed in circulation to survive a Monday list, or how long a set spends in reprocessing.

This guide works through surgical instrument sets by specialty, what each set has to contain, and how to size the quantity you actually buy.

The Quantity Question Comes First

Before any specialty list, settle the arithmetic that determines your total spend.

A set is unavailable from the moment it leaves the sterile field until it returns wrapped and cooled. That cycle — transport, decontamination, inspection, packing, sterilisation, cooling, transport back — typically runs several hours in a well-run CSSD and considerably longer where the department is a bottleneck. The working formula:

Sets required = (cases per day × reprocessing cycle in hours ÷ theatre operating hours) + 1 emergency reserve

A theatre doing four laparotomies a day with a six-hour reprocessing turnaround over a ten-hour list needs roughly three major sets in circulation, plus one held for emergencies. Buy two and the list stops at case three.

The +1 is not optional. It is the set that covers a contaminated case, a dropped instrument, or the emergency laparotomy that arrives at 2 a.m. Departments that treat the reserve as a luxury discover they have built a system with no tolerance.

General Surgery

The general surgery estate is the backbone and usually the largest line in the budget.

Major laparotomy set. The workhorse. Scalpel handles Nos. 3 and 4; Mayo and Metzenbaum scissors, curved and straight; toothed and non-toothed dissecting forceps in 15 cm and 20 cm; Adson forceps; a graded hierarchy of haemostats running Halstead mosquito, Crile, Kelly, Rochester-Pean; Allis, Babcock and Duval tissue forceps; Mayo-Hegar and Crile-Wood needle holders; Backhaus towel clamps; Poole and Yankauer suction; and retraction covering Deaver, Richardson, Morris, Langenbeck and a self-retaining Balfour.

Minor set. Roughly a third of the instrument count. Skin procedures, drainage, biopsy. High turnover, so departments need more of these than they think.

Procedure-specific sets — appendicectomy, hernia, cholecystectomy — are trimmed variants of the major set. There is a real decision here: a hospital can either stock distinct procedure sets, which cuts reprocessing volume and instrument wear, or run everything off a universal major set, which simplifies inventory but sends fifty instruments through CSSD to perform a procedure needing twenty.

High-volume centres save money with procedure-specific sets. District hospitals with unpredictable case mix are usually better served by a universal set plus supplementary singles.

Obstetrics and Gynaecology

The specialty with the least tolerance for unavailability, because a category-1 caesarean does not wait for reprocessing.

Caesarean section set — the most-used set in most hospitals. Requires a Doyen retractor for bladder retraction, Green-Armytage forceps for uterine angle haemostasis, Allis and sponge-holding forceps, heavy needle holders, and Mayo scissors. Stock deeper than the formula suggests. A unit doing five caesareans a day needs six sets, not four, because the arrival pattern is not evenly distributed.

Abdominal hysterectomy set — adds Rochester-Carmalt forceps for pedicle ligation, Wertheim or Heaney clamps for parametrial tissue, Jorgenson scissors, and a self-retaining abdominal retractor.

D&C set — Cusco or Sims speculum, single-tooth tenaculum, uterine sound, a graded set of Hegar dilators, and blunt and sharp curettes. Cheap, high-volume, and routinely under-stocked in outpatient settings.

Delivery set — cord scissors, artery forceps, episiotomy scissors, needle holder, sponge forceps. Stocked by the delivery suite rather than theatre, and frequently outside CSSD’s audit — a persistent quality gap.

Orthopaedics

Orthopaedic procurement follows a different logic because instruments are tied to implant systems.

The basic orthopaedic set is system-independent: bone-holding and reduction forceps, osteotomes and chisels, bone curettes, Leksell and Kerrison rongeurs, periosteal elevators, a mallet, bone levers, and a Gigli saw with handles.

The plating set, IM nailing set and external fixation set are not: each is matched to a manufacturer’s implant range, with drill guides, depth gauges, taps and screwdrivers that will not interchange across systems. This is the single largest trap in orthopaedic procurement. A hospital that standardises on one implant system buys one set of ancillary instruments; a hospital running three systems buys three, and stores three.

Consolidate implant systems before buying instrumentation. The instrument saving usually exceeds the negotiating advantage lost by dropping a supplier.

Note also that orthopaedic instruments take mechanical abuse no general set experiences. Rongeur jaws and osteotome edges are consumable items on a replacement cycle, not permanent assets. Budget for it.

ENT

Small instruments, high fragility, and the specialty where set discipline matters most.

Tonsillectomy set — Boyle-Davis gag with tongue blades, tonsil dissectors and pillar retractors, tonsil artery forceps, snare with wire, Negus or Denis-Browne forceps.

Septoplasty set — Killian and Cottle speculae, Freer and Cottle elevators, septum forceps, Ballenger swivel knife, bone-cutting forceps, and mallet with osteotomes.

Mastoid set — mastoid retractors, a range of curettes, micro-suction, and the microsurgical instruments that go with an operating microscope.

FESS set — Blakesley and through-cutting forceps in multiple angulations, antrum punches, sinus seekers, and curved suctions.

ENT micro-instruments — through-cutting forceps in particular — have a service life measured in cycles, not years, and their failure mode is a jaw that no longer cuts cleanly rather than a visible break. Institute a formal function check at the packing stage. Instruments that fail go for repair immediately; a blunt through-cutter tears mucosa and lengthens the procedure.

Urology, Paediatrics, Plastics, Cardiothoracic

Urology. Open sets (nephrectomy, prostatectomy) plus endoscopic sets. The binding constraint is scope reprocessing, which follows a different pathway from steel and often a different timetable. Size the endoscopic stock against that pathway, not against CSSD’s steel throughput.

Paediatrics. Not scaled-down adult sets. A paediatric major set uses shorter, finer instruments with lighter spring tension, and neonatal work needs a distinct grade again. Departments that hand surgeons an adult set with a few fine instruments added are creating a workaround, not a solution.

Plastics. Dominated by fine-instrument replacement cycles. Skin hooks, fine-tipped scissors, Adson-Brown forceps and micro needle holders are precision items whose tips lose alignment. A plastics department needs an instrument repair contract, not just a purchase contract.

Cardiothoracic. Highest unit cost per set. DeBakey and Satinsky vascular clamps, sternal saw and wire instruments, Castroviejo needle holders, coronary dilators, internal mammary retractors. Because the sets are expensive, hospitals under-buy them and then run into availability problems on a busy list. Two per active theatre is a realistic floor.

Five Procurement Mistakes That Recur

Across district hospitals and tertiary centres alike, the same errors show up in instrument tenders.

Buying sets without consulting CSSD. Theatre specifies the contents, finance signs the order, and nobody asks the department that has to reprocess it whether the tray fits the washer basket, whether the container will pass the sterilisation validation, or whether the count list can be assembled in the available packing time. Bring CSSD into the specification, not the delivery.

Specifying by instrument name alone. “Mayo scissors, 12 off” describes almost nothing. Straight or curved? 14 cm or 17 cm? Tungsten carbide inserted or plain? A tender written at that level of detail will be filled at the supplier’s discretion, and the discretion will favour the supplier’s margin.

Under-sizing the emergency reserve. Covered above, and worth repeating because it is the most common single fault. The reserve set is what stops a contaminated case from cancelling the rest of the list.

Ignoring repair capacity. A hospital that buys instruments but has no repair route ends up replacing items that needed a twenty-minute regrind. Scissors, rongeurs, needle holders and fine forceps are all economically repairable. Establish the repair channel at the same time as the supply channel.

Treating loan sets as free. Implant-linked loan instrumentation arrives without a purchase cost, but it consumes CSSD capacity, requires its own inspection at receipt, and often arrives too late for a validated cycle before the case. Count loan-set throughput against reprocessing capacity when sizing owned stock.

Single-Use Items Inside Reusable Sets

Very few sets are now purely reusable. The decision is item-by-item rather than set-by-set, and it changes the arithmetic above.

Blades, sutures and disposable suction liners are settled questions. The genuinely contested items are the ones in between: laryngoscope blades, certain retractor systems, and fine instruments whose reprocessing cost approaches their replacement cost.

Two things move a reusable item towards single-use in practice. The first is a reprocessing cost that includes hidden CSSD labour rather than just consumables — departments that cost the full cycle honestly often find low-value reusable items are uneconomic. The second is a cleaning geometry that cannot be validated, such as a long narrow lumen with no flush port.

What does not justify the switch is convenience alone. Single-use conversion moves cost from capital to recurring revenue budgets, which can look attractive in one financial year and painful in the next five. Model it over the replacement cycle, not the current budget period.

Where a set mixes both, the count list has to distinguish them explicitly. Count discrepancies frequently trace back to a disposable item that a packer expected to find in the returned tray.

What Every Set Needs Regardless of Specialty

Four requirements sit above the specialty lists, and they are where procurement adds the most value.

A written set list, versioned. Every set needs a controlled contents list with instrument names, catalogue references and quantities, held by both theatre and CSSD. Undocumented sets drift — an instrument gets added for one surgeon and becomes permanent, another goes for repair and never returns. Count discrepancies then become unresolvable. Consistent material specification. Grade the steel to the job: AISI 410 and 420 martensitic grades for cutting and load-bearing instruments where hardness is needed, 304 and 316 austenitic grades for retractors, holloware and non-cutting instruments where corrosion resistance dominates. Traceable marking. Laser-marked identifiers let a set be reconstituted after a count discrepancy and let a failed instrument be traced to its batch.

A goods-in acceptance protocol. Do not accept a set on the strength of a packing note. Check jaw closure, box joint play, ratchet engagement, scissor blade shear along the full length, and finish. The pre-purchase QC checklist sets out the full acceptance test.

Building the Procurement Schedule

Assemble the order in this sequence and the numbers hold together:

  1. Case-mix data first. Pull twelve months of theatre activity by procedure code. Buy against what the hospital actually does, not against what the specialty list says it might.
  2. Set list per procedure group. Consolidate procedures onto the smallest viable number of distinct sets.
  3. Quantity from the reprocessing formula, with the emergency reserve added per specialty.
  4. Phase the purchase. Buy the high-turnover sets to full depth first — caesarean, minor, major laparotomy — and phase the low-frequency specialty sets across subsequent budget cycles.
  5. Reserve budget for repair and replacement, allocated per specialty. Orthopaedic cutting instruments and ENT micro-instruments consume this; a major laparotomy set barely touches it.
