Day five post-op on a general surgical ward. The registrar asks the nurse to take out an abdominal wound closure, and she reaches into the dressing trolley and picks up the wrong pair.
It happens constantly. Two instruments sit side by side in almost every dressing pack, both roughly 4–5 inches, both with a blunt lower blade, both cheap enough that nobody audits which is which. But bandage scissors and suture removal scissors are not interchangeable, and using one for the other’s job either damages the instrument or hurts the patient.
The difference is entirely in the lower blade tip.
Two Blades, Two Jobs
Lister bandage scissors carry an angled blunt “foot” or probe tip on the lower blade — a flattened, upturned button designed to slide flat between a dressing and the skin without catching tissue. The blade sits at roughly 45° to the shank so the hand stays clear of the wound while cutting.
Spencer and Littauer stitch scissors carry something different: a small hook notch cut into the lower blade tip. That hook slips under a suture loop, lifts it clear of the epidermis, and presents it to the upper cutting edge. No hook means you are pressing a flat blade into a healing incision to find the loop — which is how sutures get dragged through the wound track.

Why the hook size matters
Spencer is the finer of the two stitch patterns. At 3.5 inches with a shallow notch, it handles 5-0 and 6-0 monofilament on a face or hand without the tip bottoming out against skin. Littauer at 4.5 or 5.5 inches has a deeper hook and stouter blades — the right choice for 3-0 or 2-0 closures on an abdomen or a thigh, or for braided material that sits proud of the skin surface.
Use a Littauer on a delicate facial closure and the hook lifts too much surrounding skin. Use a Spencer on a heavy 2-0 nylon and the notch either misses the loop or the blades spring under load.
The Lister Pattern in Detail
Joseph Lister’s name is attached to the bandage scissor because the design solved an antiseptic-era problem: how to remove a dressing without contaminating or traumatising the granulating surface beneath it.
The probe tip does the work. Slide it under the edge of a bandage, keep it flat against skin, and cut in short strokes advancing the foot ahead of the cutting edge. Practical notes from instrument returns we see:

- 7.25″ is the trauma-bay length. Long enough to cut through denim, boots and layered clothing in a primary survey. Some are supplied with serrated lower blades and a fluoride or black finish.
- The foot is a wear point. If the tip has been ground down during a sharpening cycle, the scissor no longer slides flat and starts nipping skin. Check it before returning instruments to circulation.
Materials and Manufacture
Both patterns are hot-forged from AISI 420 martensitic stainless, hardened to roughly 50–54 HRC at the blade. Standard-grade instruments have solid steel cutting edges. Two things separate a good pair from a poor one, and neither is visible in a catalogue photograph:
Blade set. The blades must be tensioned so they contact only at the shearing point, travelling along the cut. Loose set means the material folds instead of shearing. This is a hand-finishing operation — a bench worker closes the joint, checks the ride, and adjusts.
Hook geometry on stitch scissors. The notch has to be cut and polished, not stamped. A stamped notch leaves a burr that snags suture and, worse, scratches the wound margin.
Sharpening, Life Expectancy and When to Replace
Neither pattern is a long-life instrument by cardiothoracic standards, and neither should be treated as disposable either.
A standard-grade Lister in regular ward use will typically need re-edging once or twice before the box joint wear makes further service uneconomic. A stitch scissor in a processed set lasts considerably longer, because it cuts one thin strand at a time rather than folded layers of gauze.
The replacement triggers are specific:
- Blade separation at the tip under closing pressure. Hold the instrument to a light and close it slowly. Daylight between the blades in the last few millimetres means the set is gone.
- A ground-back probe foot. Once the blunt tip has lost its upturn, the scissor no longer runs flat under a dressing.
- A rounded or burred hook notch. On a stitch scissor this is the failure that matters, and it is invisible without magnification.
- Lateral play at the box joint. If the blades can be rocked sideways against each other, sharpening will not fix it.
Send them out in batches rather than singly — a sharpening service handles a tray of forty economically and a pair of scissors at a loss, which is generally reflected in what they charge you.
Buying and Set Composition
For a dressing trolley or a ward stock list, the practical minimum is three items: a 5.5″ Lister for general dressing work, a 7.25″ Lister for heavy material, and a 4.5″ Littauer for stitch removal. Add a 3.5″ Spencer where fine facial or paediatric closures are routine.
Bandage scissors are a high-loss item — they walk out of wards in pockets. Most hospitals buy them in tens rather than singly and accept a standard-grade product rather than TC. Stitch scissors, which live in a processed set, justify a better instrument.

Frequently Asked Questions
Can bandage scissors be used to remove sutures?
Not properly. The blunt angled foot has no hook, so it cannot lift a suture loop clear of the skin. You end up pressing the blade into the healing incision to find the strand, which drags the suture through the wound track and risks introducing surface bacteria along it. Use a Spencer or Littauer stitch pattern instead.
What is the difference between Spencer and Littauer stitch scissors?
Size and hook depth. Spencer is finer — usually 3.5 inches with a shallow notch for fine monofilament on the face, hands or in paediatrics. Littauer is 4.5 or 5.5 inches with a deeper hook and heavier blades for larger sutures on the trunk and limbs. Many catalogues list them together as “Spencer (Littauer)” which adds to the confusion.
Are bandage scissors autoclavable?
Yes. Instruments in AISI 420 stainless steel take a standard 134°C / 3-minute steam cycle without issue. Open the joint before processing, dry thoroughly, and lubricate the box joint with instrument milk on a regular cycle. Ward-stock pairs handled between patients should still go through full reprocessing rather than a wipe-down.
Why do my bandage scissors stop cutting gauze cleanly?
Usually blade set rather than sharpness. If the blades have sprung apart at the joint — commonly from cutting casting tape or a splint — they no longer contact along the shear line and the gauze folds between them. A sharpening service can re-set and re-edge them; if the box joint itself is deformed, replace the instrument.
What length bandage scissors should a trauma bay stock?
7.25 inches. That length cuts through layered clothing, denim and boot leather during a primary survey where a 5.5-inch pair stalls. Many trauma versions add serration to the lower blade to stop slick synthetic fabric sliding out of the cut.
