Toothed vs Non-Toothed Tissue Forceps: How to Choose

Toothed vs non-toothed forceps: what teeth really do to tissue, tooth ratios, and a tissue-by-tissue selection table.

There is one rule in this subject that has no exceptions: never put toothed forceps on bowel, on a blood vessel, or on a nerve.

Everything after that is judgement. And the judgement is more counterintuitive than most training makes it sound, because the instrument with teeth is frequently the gentler of the two.

What the teeth are actually doing

The intuitive model — teeth are aggressive, flat tips are kind — is wrong often enough to cause real injury.

Consider what happens when you try to hold the edge of the rectus sheath with a flat, serrated tip. The fascia is dense and slippery. To stop it sliding out, you increase closure force. That force is distributed across the whole flat tip and crushes every cell under it. You then still lose the grip, reposition, and crush a second strip of tissue.

Now do the same with a 1×2 toothed tip. The interlocking teeth mechanically capture the fascial edge. The grip comes from geometry rather than compression, so the closure force can be a fraction of what the flat tip needed. You have made two small punctures and left the rest of the tissue uncrushed.

Across dense tissue, that is a net reduction in trauma. The teeth concentrate damage into a tiny, predictable footprint instead of spreading a lower-grade crush injury over a wide area.

Invert the tissue and the logic inverts with it. On bowel serosa, a tooth is a perforation, and on an arterial wall it is an intimal injury that becomes a thrombus. There is no compensating benefit, because those tissues do not need a mechanical capture to be held — they need only enough friction to be steadied.

That is the whole of the toothed vs non-toothed forceps question in one idea: teeth trade a small, sharp, local injury for a large, diffuse one. Whether that trade is worth making depends entirely on what the tissue does when you injure it.

Toothed patterns and what the ratios mean

Toothed tips are described by their tooth count on each jaw — 1×2, 2×3, 3×4, 4×5. The lower jaw carries one fewer tooth than the upper, so they interdigitate rather than meeting point to point.

1×2 — the general workhorse

One tooth opposing two. Fine enough for skin edges, dermis, and delicate fascia, and this is the ratio you will find on most Adson and standard tissue forceps in a general set. It is the default for skin closure, and the reason “rat tooth” and “1×2” are used almost interchangeably in day-to-day theatre language.

2×3 and 3×4 — heavier grip

More teeth spread the capture across a wider bite, which suits tougher and thicker tissue: aponeurosis, thick fascia, capsule, abdominal wall closure. The individual punctures are no deeper, but there are more of them holding a heavier load.

4×5 and above — orthopaedic and tumour work

Used where the tissue is genuinely tough and the grip must not fail — periosteum, dense scar, specimen handling during tumour resection. These are not instruments for a general laparotomy tray.

Fine 1×2 with a very slender tip

Ophthalmic and plastic patterns use the same ratio on a much finer tip, so the punctures are correspondingly small. The ratio tells you the grip mechanism; the tip dimension tells you the scale.

Non-toothed patterns are not all the same

Grouping every untoothed tip together is the second big error, because two very different instruments live in that category.

Dressing forceps

Robust, flat or lightly serrated tips, no teeth. Their job is manipulating gauze, swabs, packs and dressings, and steadying loose connective tissue during dissection. They are not designed to hold living tissue securely and should not be asked to.

Atraumatic vascular tips

DeBakey and similar patterns use a fine longitudinal ribbed pattern rather than transverse serrations, arranged so that the ribs on one jaw sit between the ribs on the other. That geometry generates friction against a vessel wall without a single point of high pressure anywhere on the tip. It is the only untoothed design that genuinely holds tissue securely, and it is why DeBakey tips became standard for bowel and vessel handling rather than plain dressing forceps. The distinction matters in procurement. A tray specified as “non-toothed tissue forceps” and filled with plain dressing forceps will not do vascular work, and the surgeon will end up gripping harder to compensate — which is exactly the injury the untoothed tip was supposed to prevent.

Tissue-by-tissue decision table

In practice the toothed vs non-toothed forceps choice resolves to a single question asked at the moment of grip: does this tissue need to be captured, or only steadied? Capture requires teeth. Steadying does not, and adding teeth to it causes injury for no gain.

Four mistakes that recur

Using a toothed forceps as a retractor. Holding a wound edge open for minutes under sustained tension causes far more damage than the same instrument used in brief grips. If tissue needs holding open, use a retractor.

Assuming “atraumatic” means untoothed. It does not. Allis and Babcock forceps both hold tissue with defined geometry — Allis with a row of short interlocking teeth, Babcock with a broad fenestrated untoothed jaw — and they are selected by what tissue is being held, not by whether teeth are present. Regripping repeatedly. Every regrip is another injury. If a tip is slipping, the answer is a different pattern, not a fifth attempt with the same one.

Buying by name instead of by tip. “Adson forceps” is sold in both toothed and non-toothed versions, and a purchase order that says only “Adson 12 cm” will produce whichever the supplier ships. State the tooth ratio, or state “non-toothed, ribbed” explicitly.

What a general tray should actually hold

A laparotomy or general set that gets the toothed vs non-toothed forceps balance right usually looks like this: two fine 1×2 toothed thumb forceps at 12 cm for skin and peritoneum, two heavier 2×3 or 3×4 at 16–20 cm for fascia and abdominal wall closure, two atraumatic ribbed forceps at 16 cm and 20 cm for bowel and vessel handling, and two plain dressing forceps kept deliberately separate for swabs and packs.

The separation of that last pair matters more than it sounds. Once dressing forceps and tissue forceps are mixed in the same corner of the tray, the dressing pattern gets picked up for tissue work because it is the one closest to hand. Labelling the positions on the tray liner solves it at close to zero cost.

Length pairing is the other detail worth enforcing. A surgeon working deep in the pelvis with a 12 cm forceps has their hand inside the wound, blocking their own view. Stock both a short and a long version of each tip type rather than a single mid-length compromise.

Inspection and care

Toothed tips fail by rolling over or by losing alignment. Hold the forceps up to a light and close it gently — the teeth should mesh fully with no daylight between them and no lateral offset. A tip that meshes on one side and gaps on the other has been dropped or sprung, and it will tear rather than grip.

Non-toothed tips fail by flattening. Serrations and ribs wear smooth with use and with aggressive machine washing, and a smooth tip needs more force to hold, which is the injury the instrument existed to prevent. Check the ribbing under magnification during periodic inspection, not just at the point of purchase.

Both types should be cleaned promptly, ultrasonically processed where available, rinsed in demineralised water, dried fully and lubricated at any joint before steam sterilisation. Spring-type thumb forceps have no joint, but they do have a spring, and stacking heavy instruments on top of them in a tray is the most common cause of permanent tip misalignment.

Are toothed forceps more traumatic than smooth ones?

On dense tissue, usually less. Teeth capture tissue mechanically so far less closure force is needed, concentrating damage into two small punctures instead of crushing a wide strip. On bowel, vessels and nerves the calculation reverses completely and teeth are never acceptable.

What does 1×2 mean on a forceps?

One tooth on one jaw opposing two on the other, interdigitating when closed. It is the standard fine ratio for skin and delicate fascia. Higher ratios such as 3×4 spread the grip for tougher tissue.

Can dressing forceps be used to hold tissue?

Only for steadying loose connective tissue during dissection. They are designed for gauze, packs and swabs, and they do not hold living tissue securely — which leads the operator to squeeze harder and crush it.

Why do DeBakey tips have ribs instead of serrations?

The longitudinal ribs interlock between the jaws and generate friction along the whole tip rather than at discrete pressure points. That holds a vessel or bowel wall securely without any single spot of high compression.

How do I specify these on a purchase order?

Give the pattern name, the overall length in centimetres, and the tip specification — either the tooth ratio or “non-toothed, longitudinally ribbed”. A name and length alone will produce whichever version the supplier stocks.
