Tympanoplasty Instrument Set: Middle Ear Surgery Guide

Tympanoplasty instruments: full middle ear tray checklist, micro forceps specs, graft harvest tools, curettes and reprocessing rules.

A tympanoplasty tray is deceptive. Laid out on the Mayo stand it looks sparse next to a laparotomy set, maybe thirty pieces, most of them thinner than a pencil lead. But every one of those pieces has to work in a space roughly 15 mm across, under a microscope at 10x to 25x magnification, next to a facial nerve and a stapes footplate that tolerate no error at all.

The tray is small because the operation is precise, not because it is simple.

What the operation asks of the instruments

Tympanoplasty repairs the tympanic membrane, with or without reconstruction of the ossicular chain. Wullstein’s classification still frames the discussion: Type I is a simple membrane graft onto an intact ossicular chain (myringoplasty in practice), Types II and III involve grafting onto a remaining ossicular remnant or directly onto the stapes head, Type IV creates a cavum minor over a mobile footplate.

Graft uptake at 24 months runs around 80 percent for temporalis fascia and closer to 96 percent for cartilage in larger perforations, which is why most sets now carry both fascia-harvest and cartilage-shaping instruments rather than one or the other.

Practically, that means tympanoplasty instruments have to cover five distinct jobs: exposure of the canal, harvest of graft material, elevation of the tympanomeatal flap, work inside the middle ear at microscopic scale, and controlled bone removal at the scutum. Sets that fail in theatre usually fail because they were bought as “ear instruments” without mapping them against those five jobs.

Exposure: getting into the canal

Ear specula

Round, non-fenestrated aural specula in a graduated set, typically 4, 5, 6, 7 and 8 mm internal diameter, in 30 to 40 mm lengths. The bore should be polished internally and matte externally: internal polish reflects light down the canal, external matte kills glare back into the microscope. A speculum holder that clamps to the operating table frees the surgeon’s non-dominant hand, which is the difference between a two-handed and a one-handed operation.

Self-retaining retractors

For a postauricular approach, a Plester or Weitlaner-type self-retaining retractor with 3×4 blunt prongs holds the soft tissue envelope. Two are usually needed, placed at right angles. Endaural approaches take a smaller two-prong retractor. Blunt prongs, always; sharp prongs sit millimetres from the facial nerve trunk as it exits the stylomastoid foramen in a child.

Incision and flap elevation

A No. 3 scalpel handle with a No. 15 blade for skin. Then the specialist knives take over: a round knife and a sickle knife for the tympanomeatal flap, and a Plester or Rosen flap knife to mobilise the flap medially. The Lempert and Duckbill elevators lift canal skin. Blade widths of 1.5 mm to 3 mm cover most anatomy.

The critical quality point on these is edge geometry, not sharpness. A round knife that has been resharpened until its curve is flattened will tear the flap at the annulus every time. Instruments in this group should be inspected against a reference profile at each service interval, not just tested for sharpness.

Graft harvest

Temporalis fascia is harvested through the same postauricular incision or a separate temporal incision: a Metzenbaum or fascia scissor, toothed Adson forceps, and a fascia press or graft press to flatten and dry the harvested sheet. The press matters more than it looks. A fascia graft that is laid in wet and thick behaves differently from one pressed to a uniform thin sheet.

For cartilage grafts, tragal or conchal cartilage is taken with a No. 15 blade, then thinned. A cartilage press or cartilage slicer producing 0.3 to 0.5 mm thickness is standard; a circular cartilage cutter or trephine punch, in 6 mm to 12 mm diameters, cuts discs for shield and inlay techniques. In the inlay technique the disc is circumferentially grooved by about 1 mm so it seats into the perforation rim, which needs a clean-edged punch rather than a freehand cut.

The middle ear microinstruments

This is where the set earns or loses its reputation. Every instrument here is a fine shaft, 140 to 160 mm long, with a working tip measured in tenths of a millimetre, mounted in a knurled or flat handle sized for a pen grip.

Picks and needles

A graduated set of straight and angled picks, commonly 45 degrees, 90 degrees and 250 degrees, is used to palpate the ossicular chain, define perforation margins, and position the graft. The 90-degree pick is the instrument that tells you whether the incudostapedial joint is mobile. A Barbara micro ear needle, angled, positions delicate tissue and teases graft edges under the annulus.

Micro scissors and micro knives

Bellucci scissors, straight and angled, 2 mm to 3 mm blades on a fine shaft, cut adhesions and trim graft edges in situ. A Fisch micro dissector, double-angled, makes clean separations of soft tissue in the middle ear without the drag of a blunt elevator.

Alligator and crocodile forceps

The workhorse. Micro alligator forceps, 80 mm working length, with serrated or smooth 0.6 mm to 1.0 mm jaws, transfer everything: fascia, cartilage discs, gelfoam pledgets, ossicular prostheses, cotton. A cupped-jaw version handles gelfoam without shredding it. Buy two, because losing the only pair mid-case to a bent jaw ends the operation.

Jaw alignment is the test. Hold the forceps up to the light with the jaws closed; you should see no gap and no lateral offset. Alligator jaws are the single most commonly bent item in an ear set.

Suction

Fine metal suction tubes in 0.5, 0.7, 1.0 and 1.5 mm bores, with a finger-controlled thumb port so the surgeon can modulate suction without changing hands. Angled and malleable variants reach the sinus tympani and facial recess. Every tube needs a matching cleaning stylet in the set, and the stylet needs to be passed before sterilization, not after.

Bone removal

Access to the middle ear often requires curetting the scutum, the bony overhang at the posterosuperior annulus. A House micro ear curette, double-ended, in sizes 000 through 2, does this in a controlled way. Curettes work by pulling toward the surgeon; a curette that has to be pushed is either blunt or the wrong size.

Where trays go wrong

Four failure patterns account for most of the complaints CSSD managers raise about ear sets.

Bent alligator jaws that nobody logged

The forceps goes back in the tray, passes a visual check, and fails in the next case. Alligator forceps should be checked jaw-to-jaw under a loupe at the packing stage and the check recorded against the tray sheet. A unit doing four ears a week will bend a pair roughly every two months; if your records show none in a year, the check is not happening.

Suction tubes blocked with dried blood

A 0.5 mm bore occludes with a clot smaller than a grain of rice. If the stylet is not passed at the point of use and again at decontamination, the tube arrives sterile and useless. Keep the stylets bound to their tubes, not loose in the tray corner.

Curettes sharpened into the wrong profile

House curettes are cutting instruments with a defined cup angle. Generic sharpening services grind them like a spoon, and the instrument then skates off the scutum instead of biting. Specify profile-matched regrinding or replace them.

Mixed-vintage sets

Instruments bought piecemeal over a decade have inconsistent handle diameters and knurling. Under a microscope, where the surgeon is working by proprioception and not looking at their hands, inconsistent handles measurably slow the operation. Buy and replace ear sets as sets.

Microscopic and endoscopic sets are diverging

Endoscopic ear surgery has changed what a good set looks like. The endoscope occupies the non-dominant hand, so everything must be doable one-handed. That pushes demand toward combined suction-dissectors, suction-elevators and suction-picks, and toward slightly longer shafts, around 160 to 175 mm, so the hand clears the endoscope eyepiece.

It also removes items. A speculum and speculum holder are unnecessary once the endoscope provides both light and exposure. Retractors are needed less often because transcanal endoscopic access avoids the postauricular incision in many Type I cases.

If your unit runs both techniques, the practical answer is a shared core of tympanoplasty instruments (picks, alligators, knives, curettes) with two small approach-specific supplements, rather than two complete duplicate sets. It halves the reprocessing burden and keeps the expensive microinstruments in circulation instead of sitting in a rarely-opened tray.

Ossiculoplasty additions

If the chain is eroded, the set expands. A prosthesis measuring gauge or ossicular sizer establishes the required PORP or TORP height. A crimping forceps is needed for stapes-related work. A malleus nipper or incus replacement instruments handle the ossicles themselves. Most units keep these as a supplementary micro tray rather than adding them to every tympanoplasty pack, on the reasonable grounds that they add reprocessing burden to cases that will never open them.

Materials, handling and reprocessing

Ear micro instruments are usually AISI 420 for cutting tips and AISI 410 or 316L for shafts and non-cutting items. Titanium versions are lighter and non-magnetic, which matters for surgeons who use magnetic instrument mats, and they resist the corrosion pitting that fine tips suffer after repeated exposure to blood and saline.

Three reprocessing rules protect the set:

Use a dedicated micro instrument tray with silicone tip protectors or finger mats. Loose ear instruments in a general basket bend within a handful of cycles. The tray is not an accessory; it is part of the instrument.

Do not put fine tips through an ultrasonic bath unsupported. Cavitation is fine; instruments knocking together in the tank is not. Rack them.

Inspect under magnification, not by eye. A 0.6 mm alligator jaw that has sprung 0.2 mm out of alignment looks perfect on a bench and is useless in a middle ear. Reprocessing protocols for this class of instrument are covered in our guidance on sterilization of microsurgical instruments.

Buying a set

Tympanoplasty instruments are a category where price-per-piece is a poor guide. A cheap alligator forceps with imprecise jaws does not save money; it costs a graft. When evaluating a supplier, ask for the steel grade and hardness per item group, inspect jaw alignment and knife-edge profile on samples under magnification, and confirm that the quoted set includes the tray, tip protectors and suction stylets rather than listing them separately later.

Frequently Asked Questions

How many instruments are in a standard tympanoplasty set?

A working set runs 28 to 40 pieces depending on whether cartilage-harvest instruments and a second alligator forceps are included. Ossiculoplasty instruments usually sit on a separate supplementary tray.

What is the difference between a myringoplasty and a tympanoplasty set?

A myringoplasty set is a subset: exposure, flap elevation, graft harvest and graft placement, with no ossicular instruments. If the chain is intact and you are only closing a perforation, the smaller pack is enough and reduces reprocessing load.

Should micro ear instruments be titanium or stainless steel?

Titanium is lighter, non-magnetic and more corrosion-resistant, and many surgeons prefer it for picks and dissectors. Cutting instruments such as round knives and scissors still perform better in hardened 420 stainless, which takes and holds a finer edge.

How often should ear micro instruments be serviced?

Inspect under magnification at every reprocessing cycle and schedule a formal service every six months in an active unit. Alligator jaw alignment and round-knife edge profile are the two items that drift first.

Can endoscopic tympanoplasty use the same instruments?

Mostly yes, though endoscopic surgeons work one-handed and tend to prefer slightly longer shafts and more angled tips, and they can drop the speculum and speculum holder entirely. Suction-dissectors that combine two functions are more common in endoscopic sets.
